The filing

You sign the engagement letter between clinic sessions at the hospital you are leaving. That same afternoon your PPS specialist, who will own this file for the next four months, opens it: entity, EIN, group NPI, all sequenced on one page. By Friday the PLLC is filed with the state and the group NPI requested. You did neither. You read a two-line note saying both were done, and went back to clinic.

Licensing, in parallel

Nothing waits for anything else. The attestation

One profile feeds every payer, so your specialist builds it once and builds it clean: CV, work history, malpractice certificate, the documents gathered in week one. The first yes

Medicare answers first. The approval lands in your specialist’s morning queue on a Thursday; you get the call before lunch. It cleared on first pass, as 98% of PPS applications do, and the effective date is set. Thirteen files are still in review, each with a next-follow-up date your specialist works, not watches. You have given your notice. The far edge of the income gap is now a date on paper.

The practice opens on a Monday with twelve patients on the schedule. The EHR and clearinghouse were wired in week nine, before the doors, not after, so the first visit’s claim is scrubbed and out the same night. You spend opening day with patients. It is a Tuesday again. The first remittance posts: the opening-week claims, paid clean on first pass, no adjustments. One hundred and eighteen days from engagement letter to money moving into the practice's own account, and every one of those days is in this file: dated, stamped, owned by someone whose job was to move it.

Credentialing a provider commonly runs 45 to 90 days with commercial payers. Contracting — the executed agreement and a live effective date — is the half nobody can schedule: a closed panel, an appeal against it, or terms that have to be negotiated can add months, and it moves differently in every state. Anyone quoting you one flat number is quoting a sales figure. Ask them to be paid only when your effective dates actually land, and watch how fast the number changes.
